- Identify the cause of the error.
- Check your forms 1095-C to verify if Line 14, under Part II, contains the required data.
- Line 14 should have an Offer of Coverage code either in the All 12 Months column or in every one of the 12 monthly columns.
- If the All 12 Months column does NOT have an Offer of Coverage Code, an error will occur with the IRS if any of the other 12 columns on Line 14 are also blank.
- If the All 12 Months column has an Offer of Coverage Code, an error will occur with the IRS if any of the other 12 columns on Line 14 also contain an Offer of Coverage code.
- If Line 14 of Form 1095-C is incorrect, it is likely that the ACA Timeline for the same employee.
- The most common cause of the ACA Timeline being incorrect for the Line 14 data is that the Benefit Profile was not properly assigned to the applicable employee.

- Line 14 should have an Offer of Coverage code either in the All 12 Months column or in every one of the 12 monthly columns.
- Check your forms 1095-C to verify if Line 14, under Part II, contains the required data.
- Correct the error in Fuse.
- Update the Employee settings/data if applicable.
- Update the forms 1095-C that do not have Line 14 populated correctly
